  2. Ask if patient wears or has a history of wearing: Oral Sprint or Night Guard
  3. Medications
  4. Has the patient ever seen a TMJ Specialist
  5. Has the patient had Jaw surgery
  6. Occlusal Instability Vertical Closure, open bite, Deep bite (Facial of Lowers in contact with Lingual of Uppers)
  7. Jaw Opening-Right
  8. Jaw Opening-Left
  9. Jaw Opening-Protrusion
  10. Muscular Palpation
  11. Crepitus Open Slightly
  12. Crepitus Open Wide
  13. Click Open Slightly
  14. Click Opens Lightly
  15. Click Open Wide
  16. Pain On Load Testing
  17. Evaluate Wear Patterns Anterior Edge to Edge & Lateral Guidance
    • Anterior Wear Pattern Daytime Parafunction
    • Wear Lateral Guidance wear pattern can be from either or both: Daytime Parafunction or Nighttime Bruxism (* Nighttime Bruxism is statistically linked to Sleep Apnea events).
