Orthodontic Treatment Times & The Predictability of Results Can Vary With
Patient Cooperation
Patient cooperation is one of the most essential factors in determining whether treatment successful and is completed on time.
To Help Achieve The Most Predictable And Successful Results In A Timely Fashion Patients Must:
- Keep regularly scheduled appointments.
- Practice good oral hygiene, including brushing, flossing, etc.
- Wear orthodontic appliances & clear teeth aligners all the time unless you are eating or brushing and flossing your teeth
- Wear rubber bands (elastics) as instructed & if necessary.
- Eat proper foods so as not to dislodge the braces (brackets, bands).
- Schedule in a timely fashion if braces, brackets come loose or break or if you loose clear teeth aligners or appliances
- Wear retainers after braces are removed.
- Willingness to choose and wear recommended appliances like palatal expanders, brackets or buttons or cross arch elastics.
- Willingness of the patient to choose and invest in the appropriate treatments as it relates to their existing condition. I.e. if a patient has severe teeth alignment problems, treatment will be faster and more predictable with surgical orthodontic treatments.

Severity Of The Problem
If boney changes are required to achieve the smile of your dreams, this will extend the treatment times. I.e. need to move the bone in order to set the base for the newly aligned tooth root positions. Examples:
- Patient who has a narrow mouth and is seeking a wider smile -will add time to treatment if the top of the mouth (palate) will need to be expanded.
- Some specific types of tooth movements take longer i.e.
- tooth rotation takes longer then tooth tipping or torqueing.
- Ectopic Eruption i.e. a need to move the entire body of the tooth: both the root and crown of a tooth.
- Underbite / Cross bite-Lower teeth on the outside of upper teeth
- Severity of Misalignment. I.e. if one tooth is only slightly out of position, treatment will be quicker, verses if all teeth are crooked or severely Malaligned will take longer.
- Unraveling severely crowded or overlapping teeth with no gaps or space between teeth is more difficult, verses straightening teeth when there is open space to move the teeth into i.e. pretreatment spaces or gaps between teeth leaves space to move the teeth into the direction of desired alignment.
Braces Vs. Invisalign -Type of treatment the patient chooses will effect time of treatment.
While Invisalign can be a fast treatment choice, it’s not always the fastest choice for many types of tooth movements. Its very well known to experienced dentists and orthodontist that severely misaligned teeth can be more quickly and more predictably moved with braces and fixed appliances.

Each Person’s unique Genetic makeup
Each person’s unique response to the orthodontic treatment will effect treatment times.
- Some patients have harder bone that requires a longer treatment, some have soft bone faster treatments.
- Young /Old &
- Male / Female
- Strong facial bone shape / petite facial bone shape
- Airway problems i.e. Tongue position or tongue thrust movements or habits can slow or can even stop tooth movement.
- Physical and dental health (periodontal disease)
- Parafunctional habits –daytime habits or nighttime habits
Age of patient – Teeth move fast in younger patients who are still growing, and slower in a more mature patient who has bone that is typically mature or hardened.
The normal treatment time with braces or invisalign is about 24 to 30 months. However, this can vary considerably based on the factor shared above.

This time period does not include “phase I” treatment or the “orthopedic phase” (where the orthopedic appliances are utilized while some of the primary or “baby teeth” are still present).

Tooth Retention adds time to the end of orthodontic treatments
During the final stages of orthodontic treatments teeth become aligned and patients often prematurely request that the braces get removed. Please understand, there are two phases of orthodontic treatment: active movement phase and the phase of holding the teeth in position while the tooth supporting bone regrows around the tooth roots. While it may seem that your teeth are aligned and that you are finished, if the appliances are removed too quietly the bone does not have time to remodel / firm up and the teeth will likely move out of ideal alignment. Good news about retention. If a patient is responsible, quite often after the teeth look aligned, we will quickly remove the active orthodontic appliance and position a “removable clear teeth retainers.”
